Companies operating a contract crew transportation vehicle must, at the request of the commission, provide data relevant to any complaints and accidents, including:

1. (1) Location;
2. (2) Time of day;
3. (3) Visibility;
4. (4) Description of the event;
5. (5) Any resulting property damage or personal injuries;
6. (6) Any corrective action taken by the railroad company, person operating the contract crew transportation vehicle, or the commission.
