Each petition for a ruling on a nonassociation claim shall contain, in separate numbered paragraphs:

1. (1) Information identifying the parties and (if known) their representatives, including:

   1. (a) The name, address and telephone number of the employer, and the name, address, telephone number, fax number, and email address of its principal representative;
   2. (b) The name, address and telephone number of the employee organization, and the name, address, telephone number, fax number, and email address of its principal representative; and
   3. (c) The name, address, telephone number, fax number, and email address of the employee who has asserted a right of nonassociation, and the name, address, telephone number, fax number, and email address of his or her representative.

2. (2) Indicate the matters in dispute as including:

   1. (a) The eligibility of the employee to assert a right of nonassociation; and/or
   2. (b) The designation of the charity to receive the alternative payments.

3. (3) Indicate whether the petition is filed on behalf of:

   1. (a) The employee; or
   2. (b) The employee organization.
4. (4) The name, signature and, if any, title of the person filing the petition, and the date of the signature.

5. (5) Information concerning the parties' relationships, including:

   1. (a) The employer's principal business;
   2. (b) A copy of the current (or most recent) collective bargaining agreement; and
   3. (c) A description of the bargaining unit involved, specifying inclusions and exclusions.

6. (6) Indicate whether the claimed right of nonassociation is based upon:

   1. (a) Personal religious beliefs; or
   2. (b) Upon the teachings of a church or religious body identified in the petition, including the name, address, telephone number, fax number, and email address of its contact person.
7. (7) The name(s) of the charity or charities to which the petitioner proposes to make alternative payments, including the name(s), address(es), telephone number(s), fax number(s), and email address(es) of its/their contact person(s).
8. (8) Indicate whether disputed funds are being held in escrow by the employer.
9. (9) Any other relevant facts.
