Except for those services described in WAC 388-891A-1110, DVR provides and pays for VR services when services are not available to you from, or paid for by, any other program administering or providing comparable services or benefits, as described in WAC 388-891A-0010(1), and:

1. (1) You have completed the application requirements;
2. (2) You have provided documents that verify your identity and legal work status;
3. (3) DVR authorizes the services before the services begin; and

4. (4) You have completed the financial statement, if required, and have agreed upon what portion, if any, you are required to contribute for your VR services, and:

   1. (a) You need the services so that a DVR counselor can determine your eligibility for services; or

   2. (b) You need the services to assist you and your DVR counselor as you:

      1. (i) Identify your vocational rehabilitation needs; or
      2. (ii) Prepare for, secure, maintain, advance in, or regain employment.
