DVR does not pay for any of the following medical treatments other than as an exception to policy, as described in WAC 388-891A-0205:

1. (1) Maintenance of your general health or fitness;
2. (2) Cosmetic procedures, such as facelifts, liposuction, and cellulite removal;
3. (3) Maternity care;
4. (4) Hysterectomies, elective abortions, sterilization, and contraceptive services as independent procedures;
5. (5) Drugs not approved by the U.S. Food and Drug Administration for general use or by state law;
6. (6) Life support systems, services, and hospice care;
7. (7) Transgender services including surgery and medication management;
8. (8) Homeopathic and herbalist services, Christian Science practitioners, or theological healers;
9. (9) Treatment that is experimental, obsolete, investigational, or otherwise not established as effective medical treatment.
