You have completed the application requirements for VR services when you:

1. (1) Have provided information needed to begin an assessment of eligibility and priority for services;
2. (2) Are available to participate in assessment services necessary to determine if you are eligible for VR services; and

3. (3) Have signed an application form provided by DVR or otherwise provided a written request that includes the following information:

   1. (a) Your name;
   2. (b) Your contact information, including mailing address and phone or email as appropriate;
   3. (c) Your birth date and gender;
   4. (d) Your Social Security number (optional); and
   5. (e) The date on which you signed the written request.
