- (2) The department requires a written recommendation from a behavioral health or related provider. The recommendation must include a description of your need for the service and take into account that the service is expected to complement the existing behavioral health support plan.
- (3) DDA may require a second opinion by the department-selected provider.
- (4) Equine therapy services must not exceed the CIIBS combined specialized-hourly services allocation of $5,000 per waiver year.
- (5) Equine therapy services must not be used to provide hippotherapy, which is an occupational therapy service.
- (6) The department reserves the right to terminate the authorization for equine therapy services if there is not a demonstrable improvement in behavior as documented by the contracted equine therapist or other treatment provider.
