1. (1) Extermination of bedbugs is available on the following waivers:

   1. (a) Core;
   2. (b) Basic plus; and
   3. (c) Community protection.

2. (2) Extermination of bedbugs services covers only:

   1. (a) The assessment or inspection by the qualified provider;
   2. (b) The application of chemical-based pesticide or heat treatment; and
   3. (c) One follow-up visit.
3. (3) Extermination of bedbugs is limited to two treatment cycles per plan year.

4. (4) Extermination of bedbugs excludes:

   1. (a) Lodging during the extermination process; and
   2. (b) Preparatory housework associated with the extermination process.

5. (5) DDA does not cover extermination of bedbugs for a participant who lives:

   1. (a) With their family; or
   2. (b) In an adult family home, assisted living, group home, group training home, licensed staffed residential home, or other facility contractually obligated to provide housing.
