DDA must immediately decertify a companion home provider if the provider or another person age sixteen or older living in the companion home:

1. (1) Has been convicted of, or has a pending charge for a crime that is disqualifying under chapter 388-113 WAC; or
2. (2) Has a negative action under WAC 388-825-640.
