1. (1) The provider must maintain a provider profile that discloses:

   1. (a) The name and age of any person living in the home besides the provider;
   2. (b) The number and type of pets living in the home;
   3. (c) Other obligations (e.g., employment, volunteer work, caring for a dependent) the provider has that may impact the client's choice of provider.

2. (2) The provider must send the provider profile to:

   1. (a) A potential client at initial client contact; and
   2. (b) DDA at time of application and any time a change occurs.
