The companion home provider must participate in the certification evaluation process with DDA staff and DDA-contracted evaluators by:

1. (1) Allowing scheduled and unscheduled home visits;
2. (2) Providing information and documentation as requested;
3. (3) Cooperating in setting up appointments;
4. (4) Responding to questions or issues identified; and
5. (5) Participating in an exit conference.
