The purpose of the person-centered service plan is to create a written plan that includes:

1. (1) Your goals and desired outcomes;
2. (2) The services and supports, both paid and unpaid, that will assist you to achieve your identified goals;
3. (3) Your acuity scores generated from the support assessment;
4. (4) Referral information;
5. (5) The SSP, if any, you are approved to receive in lieu of a DDA paid service;
6. (6) DDA paid services you are authorized to receive; and
7. (7) If you are enrolled in a DDA waiver, your PCSP must address all of your assessed health and welfare needs.
