1. (1) DDA may extend a provider's certification period if an evaluation cannot be completed timely. DDA must notify the provider of their new certification due date.
2. (2) The provider's performance during the extension period is reviewed as part of the provider's evaluation.
3. (3) The provider's new certification period begins at the end of the extension period.
