When determining the length of a provider's certification, DDA considers the following:

1. (1) Whether it is a provider's first evaluation for initial certification;
2. (2) Number of citations received during certification evaluations or other DDA reviews;
3. (3) Category of citations issued; and
4. (4) Whether a repeat citation is issued.
