The provider must participate in the certification evaluation process with DDA employees and DDA-contracted evaluators by completing actions such as:

1. (1) Participating in scheduled and unscheduled visits;
2. (2) Cooperating in scheduling visits;
3. (3) Providing information and documentation as requested;
4. (4) Responding to questions or issues identified;
5. (5) Participating in an exit meeting; and
6. (6) Implementing a corrective action plan.
