1. (1) To be considered eligible with autism:

   1. (a) You must be diagnosed with:

      1. (i) Autism spectrum disorder; or
      2. (ii) Autistic disorder before February 1, 2022;
   2. (b) The condition must have originated before age 18; and

   3. (c) You must have been diagnosed by:

      1. (i) A licensed neurologist;
      2. (ii) A licensed psychiatrist;
      3. (iii) A licensed psychologist;
      4. (iv) A licensed developmental and behavioral pediatrician;
      5. (v) A center of excellence as defined in WAC 182-531A-0200; or

      6. (vi) One of the following professionals associated with an autism center or developmental center:

         1. (A) A licensed physician;
         2. (B) An ARNP;
         3. (C) A physician assistant; or
         4. (D) A naturopathic physician.
2. (2) The diagnosis must be documented in a diagnostic report.
