Wash. Admin. Code § 388-823-0105 – How will DDA notify me of the results of my eligibility determination?

(1) DDA will provide written and verbal notification of the determination.
(2) If you are determined eligible, the written notification will include:
1. (a) Your eligible condition(s);
2. (b) The effective date of your eligibility;
3. (c) The expiration date or review date of your eligibility, if applicable; and
4. (d) The name and phone number of your DDA primary contact.
(3) If you are determined not eligible, the written notification will:
1. (a) Explain the decision;
2. (b) Explain your appeal rights to the decision; and
3. (c) Provide you with an administrative hearing request form.
