Wash. Admin. Code § 388-823-0020 – How do I enroll with the developmental disabilities administration?

(1) You may enroll with the developmental disabilities administration (DDA) if you apply for eligibility with DDA and DDA determines that you meet all eligibility criteria required to establish a developmental disability as defined in this chapter.
(2) You apply to become a client of DDA by contacting a DDA office and requesting a DDA eligibility packet. You may also download, complete, and return an eligibility packet at https://www.dshs.wa.gov/dda/consumers-and-families/eligibility.
(3) You must complete and return the required forms, along with any supporting documentation that you have.
