- (1) You may enroll with the developmental disabilities administration (DDA) if you apply for eligibility with DDA and DDA determines that you meet all eligibility criteria required to establish a developmental disability as defined in this chapter.
- (2) You apply to become a client of DDA by contacting a DDA office and requesting a DDA eligibility packet. You may also download, complete, and return an eligibility packet at https://www.dshs.wa.gov/dda/consumers-and-families/eligibility.
- (3) You must complete and return the required forms, along with any supporting documentation that you have.
