1. (1) The department may deny a registration application when the department determines the provider applicant does not:

   1. (a) Meet the registration requirements and operational standards of WAC 388-116-3010; or
   2. (b) Meet the application requirements in WAC 388-116-3050.

2. (2) The department will notify the provider applicant in writing when a denial decision has been made and include the following in the notification:

   1. (a) Date of denial;
   2. (b) Reason for denial;
   3. (c) How to contact the department with questions; and
   4. (d) Instructions on how to request an administrative hearing to contest the denial decision.
