1. (1) In order to change the licensee of an enhanced services facility, the current licensee must notify the following in writing of the proposed change in licensee:

   1. (a) The department; and
   2. (b) All residents, or resident representatives (if any).

2. (2) The licensee must include the following information in the written notice:

   1. (a) Name of the present licensee and prospective licensee;
   2. (b) Name and address of the enhanced services facility for which the licensee is being changed; and
   3. (c) Date of proposed change.
