Before the enhanced services facility employs, directly or by contract, an administrator, staff person or caregiver, or accepts any volunteer, or student, the home must:

1. (1) Require the person to complete a DSHS background authorization form; and
2. (2) Submit to the department's background check central unit, including any additional documentation and information requested by the department.
