The enhanced services facility must:

1. (1) Keep the records of TB test results, reports of chest radiograph findings, and any physician or health care provider orders in the enhanced services facility;
2. (2) Make the records readily available to the appropriate health provider and licensing agency;
3. (3) Retain the records for at least two years after the date the staff either quits, is terminated, or released; and
4. (4) Provide the staff with a copy of his/her test results.
