Wash. Admin. Code § 388-101D-0535 – Crisis diversion bed and support service providers—Client records

(1) Crisis diversion bed and support service providers must keep the following information in client records:
1. (a) Client's name, address, and Social Security number;
2. (b) Name, address, and telephone number of the client's relative or legal representative; and
3. (c) Progress notes and incident reports on clients.
