The service provider must:

1. (1) Review and revise the individual instruction and support plan as goals are achieved or as client assessed needs change in order to reflect the client's current needs, goals, and preferences:

   1. (a) At least semi-annually; and
   2. (b) At any time requested by the client or the client's legal representative.
2. (2) Send an updated copy of the instruction and support goals of the individual instruction and support plan and the list of applicable support and service information and where the information is located to the case manager for review.
