Before allowing a community protection program client to live in the same household with supported living clients who are not in the community protection program, the service provider must:

1. (1) Provide the department with a written description of the household composition;
2. (2) Participate with the treatment team during the household composition review;
3. (3) Obtain written approval from the division of developmental disabilities; and
4. (4) Obtain written consent from each client in the household or the client's legal representative if the client is unable to consent.
