If the service provider employs professional staff, the service provider must have professional liability insurance or errors and omissions insurance. The insurance must include:

1. (1) Coverage for losses caused by errors and omissions of the service provider, its employees, and volunteers; and

2. (2) Minimum limits of:

   1. (a) Each occurrence - One million dollars; and
   2. (b) General aggregate - Two million dollars.
