1. (1) A service provider:

   1. (a) May contest a decision made by the department pursuant to chapter 71A.12 RCW and according to the provisions of chapters 34.05 RCW and 388-02 WAC;
   2. (b) Must file any request for a hearing with the office of administrative hearings at the mailing address specified in the notice of imposition of an enforcement remedy; and
   3. (c) Must make the request within twenty-eight days of receipt of the written notice of the department's certification action.
2. (2) Except for the imposition of civil penalties, certification actions are effective immediately upon notice and will continue pending any hearing.
