The service provider must:

1. (1) Submit a signed plan of correction to the department according to established department processes and timelines; and

2. (2) Include in the plan of correction:

   1. (a) What the service provider did or will do to correct each deficiency;
   2. (b) How the service provider will prevent future problems of this type;
   3. (c) Who will be responsible for monitoring the corrections to ensure the problems do not recur; and
   4. (d) When lasting correction will be achieved.
