1. (1) The license year is one year from date of issue. License fees are paid annually. Fees shall be as follows:

   	Wrestling participant	-	$35.00
   	Inspector	-	$75.00
   	Event physician	-	No charge
   	Promoter	-	$220.00
   	Theatrical wrestling school	-	$540.00

2. (2) No license fee is required for persons licensed under chapter 36-12 or 36-14 WAC as an inspector, event physician or promoter.

3. (3) In addition to license requirements found in chapter 67.08 RCW:

   1. (a) Wrestling participants shall submit a small photograph of themselves that is not more than two years old.

   2. (b) Theatrical wrestling schools:

      1. (i) Must provide proof of having an established place of business that offers training in theatrical wrestling.
      2. (ii) Must provide proof of having an active tax registration through the department of revenue.
      3. (iii) Must reapply for licensure when there is a change in the location or change in ownership.
