What must my appeal contain? An application for an adjudicative proceeding before the department under RCW 34.05.413 through 34.05.494 must:

1. (1) Be in writing;
2. (2) Be signed by the applicant or the applicant's representative;
3. (3) Specify the factual basis for appeal and the issue(s) to be adjudicated in the proceeding; and
4. (4) Include a copy of the department's written determination that is the subject of the appeal.
