1. (1) If you are returning to active status from less than five years of inactive status, submit to the board:

   1. (a) A letter of application requesting reactivation of your license; and
   2. (b) The current renewal fee.

2. (2) If you are returning to active status after five years of inactive status, submit to the board:

   1. (a) A letter of application requesting reinstatement; and
   2. (b) The current renewal fee plus the late penalty fee.
