1. (1) You must provide training and information to employees exposed to formaldehyde at all of the following times:

   1. (a) At the time of initial assignment to a work area where there is formaldehyde exposure.
   2. (b) Whenever there is a new exposure to formaldehyde in their work area.
   3. (c) At least every twelve months after initial training.

2. (2) You must make sure training includes at least the following:

   1. (a) The contents of this chapter and SDS for formaldehyde.
   2. (b) The purpose of medical evaluations and a description of how you are fulfilling the medical evaluation requirements of this chapter.

   3. (c) The health hazards and signs and symptoms associated with formaldehyde exposure, including:

      1. (i) Cancer hazard.
      2. (ii) Skin and respiratory system irritant and sensitizer.
      3. (iii) Eye and throat irritation.
      4. (iv) Acute toxicity.
   4. (d) How employees will immediately report any signs or symptoms suspected to be from formaldehyde exposure.
   5. (e) Descriptions of operations where formaldehyde is present.
   6. (f) Explanations of safe work practices to limit employee exposure to formaldehyde for each job.
   7. (g) The purpose, proper use, and limitations of personal protective clothing.
   8. (h) Instructions for the handling of spills, emergencies, and clean-up procedures.
   9. (i) An explanation of the importance of exposure controls, and instructions in the use of them.
   10. (j) A review of emergency procedures, including the specific duties or assignments of each employee in the event of an emergency.
   11. (k) The purpose, proper use, limitations, and other training requirements for respiratory protection, as required by a separate chapter, Respirators, chapter 296-842 WAC.
3. (3) You must make sure any written training materials are readily available to your employees at no cost.
