1. (1) The employer must designate the refinery manager as the person with authority and responsibility for compliance with Part B of this chapter.
2. (2) The employer must develop, implement, and maintain an effective written process safety management (PSM) program, which must be reviewed and updated at least once every three years.
3. (3) The employer must develop, implement, and maintain an organizational chart that identifies management positions responsible for implementing the PSM program elements required by Part B of this chapter.
4. (4) The employer must develop, implement and maintain an effective program to track, document, and assess leading and lagging process safety performance indicators.
