Applicants must provide the following information to the department:

1. (1) The reason explosives or blasting agents will be used;
2. (2) The location where explosives or blasting agents will be used;
3. (3) The kind of explosives or blasting agents to be used;
4. (4) The amount of explosives or blasting agents to be used;

5. (5) An explosives storage plan:

   1. (a) Documenting proof of ownership of a licensed storage magazine; or
   2. (b) With a signed authorization to use another person's licensed magazine; or
   3. (c) With a signed statement certifying that the explosives will not be stored.
6. (6) An authorized agent list, if the purchaser chooses to authorize others to order or receive explosives on their behalf;
7. (7) The identity and current license of the purchaser's blaster;
8. (8) Information required by WAC 296-52-61010, License applicants must provide this information;
9. (9) Any other pertinent information requested by the department.
