1. (1) No objective findings. Subjective complaints may be present or absent.
2. (2) Anatomical or functional loss of one testicle.
3. (3) Anatomical or functional loss of both testes after the age of 65.
4. (4) Anatomical or functional loss of both testes between the ages of 40 and 65.
5. (5) Anatomical or functional loss of both testes before the age of 40.
