1. (1) You must send us the following information about yourself and each of your Washington dealers when you are certified:

   1. (a) Dealership name;
   2. (b) Mailing and physical address;
   3. (c) Phone and fax number if available;
   4. (d) Type of recreational park trailer(s); and
   5. (e) Contact person.
2. (2) You must update this information as it changes.
