1. (1) An applicant for physical therapist license shall submit to the department:

   1. (a) A completed application;
   2. (b) The application and licensing fees required under WAC 246-915-990;
   3. (c) An official transcript from CAPTE; and
   4. (d) Verification of passing NPTE scores.

2. (2) An applicant for physical therapist assistant license shall submit to the department:

   1. (a) A completed application;
   2. (b) The application and licensing fees required under WAC 246-915-99005;
   3. (c) An official transcript from CAPTE; and
   4. (d) Verification of passing NPTE scores.
