An applicant seeking waiver of the education and experience requirements as provided in RCW 18.59.070(3) shall submit the following as proof of actual practice:

1. (1) Applicant's affidavit containing the following information:

   1. (a) Location and dates of employment between June 7, 1981 and June 7, 1984;
   2. (b) Description of capacity in which applicant was employed, including job title and description of specific duties;
   3. (c) Description of nature of clientele; and
   4. (d) Name and title of direct supervisor.
2. (2) Written job description.

3. (3) Affidavit from employer(s), from June 7, 1981 through June 7, 1984, containing the following information:

   1. (a) Dates of applicant's employment,
   2. (b) Description of applicant's specific duties, and

   3. (c) Employer's title.

      After reviewing the information submitted, the board may require submission of additional information if the board deems additional information necessary for purposes of clarifying the information previously submitted.

      The proof of actual practice shall be submitted to the board's office no later than March 1, 1985.
