- (1) A dental therapist shall submit a complete report of any patient mortality or other incident which results in temporary or permanent physical or mental injury requiring hospitalization of said patient during, or as a direct result of dental procedures or anesthesia.
- (2) A dental therapist shall notify the commission in writing within 72 hours of discovery and must submit a complete written report to the commission within 30 days of the incident.
