1. (1) The department will screen applications for the following criteria:

   1. (a) Received in the Office of Community and Rural Health, P.O. Box 47834, Olympia, Washington 98504-7834, on or before the due date.
   2. (b) One original application and two unbound copies provided, sufficiently legible to be copied. The department will determine legibility; and
   3. (c) Application contains each of the items described in WAC 246-560-060.
2. (2) Applications that contain all screening criteria will be reviewed.
3. (3) If an application fails to contain any screening criterion, it will not be reviewed. The applicant will be notified in writing.
