1. (1) To receive an informational copy of a birth or death record, the applicant must submit to the state or local registrar:

   1. (a) Information to correctly identify the record on a form provided by the state or local registrar consistent with the requirements of (c) or (d) of this subsection; and
   2. (b) Fees required by RCW 70.58A.560 and WAC 246-491-990;

   3. (c) For an informational copy of a birth record, the following information as it appears on the record:

      1. (i) First, middle, and last name of the subject of the record;
      2. (ii) First and last name of all parents listed on the record;
      3. (iii) Date of birth; and
      4. (iv) City or county where the birth occurred.

   4. (d) For an informational copy of a death record, the following information:

      1. (i) First and last name of the decedent;
      2. (ii) Approximate date of death; and
      3. (iii) City or county where the death occurred.
2. (2) For the purpose of this section, "birth" includes delayed birth.
