The LOSS owner who is permanently abandoning any portion of a LOSS, including a septic tank, seepage pit, cesspool, or other sewage container, and all accessible parts of the drainfield that may become a hazard, including distribution boxes and pump chambers, shall provide:

1. (1) Written notice of the abandonment to the department and local health jurisdiction of the county where the LOSS is located a minimum of one year in advance;
2. (2) Department-approved, return receipt requested written notice of the abandonment to all affected customers and property owners whose connected dwellings, structures, lots, or units will no longer have sewage service a minimum of one year in advance;
3. (3) Hire an approved pumper to remove all partially treated sewage;
4. (4) Remove all mechanical and electrical equipment;
5. (5) Remove or destroy all lids;
6. (6) Fill any voids with soil or gravel; and
7. (7) Meet any other local or state requirements.
