The registrant must maintain the following records on-site and make them available to the department:

1. (1) Training records for each operator must be maintained for five years. Training records must document:

   1. (a) Operator's name;
   2. (b) Date of training; and
   3. (c) Training curriculum provided.

2. (2) Radiation surveys conducted by a qualified expert must be maintained for five years. The following information must be documented:

   1. (a) Security screening system make, model, serial number, and facility location;
   2. (b) Name of qualified expert who completed the survey;
   3. (c) Survey date;
   4. (d) Make, model, serial number, and calibration dates of instrumentation used to conduct the survey;
   5. (e) Results of the visual inspection of the security screening system safety interlocks;
   6. (f) Background measurements;
   7. (g) Radiation survey measurements;
   8. (h) Survey diagram, including:
   9. (i) Security screening system parameters at which measurements were made; and
   10. (ii) Drawings must be to scale as applicable.

3. (3) Maintenance logs must be maintained for the life of the security screening system. Logs must document:

   1. (a) Upgrades;
   2. (b) Modifications;
   3. (c) Maintenance or repairs made; and
   4. (d) Replacement.

4. (4) Repeat screening log must be maintained for five years. Repeat screening log must be completed any time an operator cannot determine if an individual is concealing contraband from the initial screening, and additional screening occurs. The following must be documented:

   1. (a) Name of operator conducting the screening;
   2. (b) Name of individual screened;
   3. (c) Number of repeated screens performed; and
   4. (d) Justification for conducting the repeat screening.
