1. (1) When property is determined contaminated, the local health officer shall report the contaminated property to the state department of health:

   1. (a) By telephone or email within one working day; and
   2. (b) In writing within ten working days.

2. (2) The local health officer's written contamination report to the state department of health shall include:

   1. (a) Description of the findings;
   2. (b) Conclusions;
   3. (c) Name of the property owner;
   4. (d) Mailing and street address, including zip code and county, of the property owner;
   5. (e) Parcel identification number and legal description of the property to including township and section;
   6. (f) Tax account number; and
   7. (g) Date property determined contaminated.
