1. (1) A laboratory director shall provide the following information in each laboratory report required under this chapter:

   1. (a) Patient's first and last name;
   2. (b) Patient's physical address including zip code;
   3. (c) Patient's date of birth;
   4. (d) Patient's sex;
   5. (e) Patient's ethnicity, as required in WAC 246-101-011(4);
   6. (f) Patient's race, as required in WAC 246-101-011(5);
   7. (g) Patient's preferred language, as required in WAC 246-101-011(6);
   8. (h) For hepatitis B virus, pregnancy status (pregnant, not pregnant, or unknown) of patients twelve to fifty years of age;
   9. (i) Patient's best contact telephone number;
   10. (j) For blood lead level, medicaid status of patients less than seventy-two months of age;
   11. (k) Requesting health care provider's name;
   12. (l) Requesting health care provider's phone number;
   13. (m) Address where patient received care;
   14. (n) Name of submitting laboratory;
   15. (o) Telephone number of submitting laboratory;
   16. (p) Specimen type;
   17. (q) Specimen collection date;
   18. (r) Date laboratory received specimen;
   19. (s) Test method used; and
   20. (t) Test result.
2. (2) The local health officer or the state health officer may request additional information of epidemiological or public health value when conducting a case investigation or otherwise for prevention and control of a specific notifiable condition.
