Bingo licensees must imprint at least the following on customer receipts:

1. (1) The date; and
2. (2) The name of the licensee operating the activity; and
3. (3) The separate amount of money paid to play each type of game; and
4. (4) The total amount of money paid; and
5. (5) The consecutive customer receipt number.
