1. (1) To be eligible for the kidney center services described in this section, a person must be diagnosed with end-stage renal disease (ESRD) or acute renal failure and be covered under:

   1. (a) One of the Washington apple health programs listed in the table in WAC 182-501-0060;
   2. (b) Alien emergency medical; or
   3. (c) Qualified medicare beneficiary (QMB) - (The agency pays only for medicare premium, coinsurance and deductible).
2. (2) Managed care enrollees must have dialysis services arranged directly through their designated plan.
