1. (1) Upon completion of the internship, the intern shall submit to the school district a report that includes the following information:

   1. (a) Completion of the requirements under WAC 181-83-030;
   2. (b) Summary evaluation by the intern of the internship experience;
   3. (c) Summary evaluation by the business, industry, or government agency representative of the intern's experience; and
   4. (d) Whether the internship will be claimed for purposes of recognition on the salary allocation schedule developed by the legislative evaluation and accountability program committee.
2. (2) The report shall be signed by the intern and business, industry, or government agency representative.
3. (3) The school district shall forward annually to the professional educator standards board, on or before December 1, the internship reports for the previous school year.
