1. (1) The name of the general permit under which coverage is being sought, and a statement declaring the applicant's intent to comply with the requirements of the permit.
2. (2) The activities conducted by the applicant that require it to obtain coverage.
3. (3) Name, mailing address, and location of the facility for which the application is submitted.
4. (4) The operator's name, address, telephone number, ownership status, and status as federal, state, private, public, or other entity.
5. (5) The location and a description of any site(s) where biosolids or sewage sludge are treated, stored, disposed, or applied, and whether or not any permit, including a local solid waste permit has been issued for a site.
6. (6) Any information specifically required for a notice of intent under the applicable general permit.
