1. (1) A conservation district must create a ballot and provide a ballot to each person who requests a ballot to vote in the conservation district election.

2. (2) The official ballot must:

   1. (a) List the name of each candidate the conservation district has verified as eligible;
   2. (b) List names on the ballot in alphabetical order by last name, from top to bottom, with an empty checkbox or blank space next to each candidate's name; and
   3. (c) Contain at least one blank line where a voter can enter the name of a write-in candidate.
