1. (1) All serious violations and violations listed in WAC 388-730-0070(2) must be documented in an incident report. The incident report must include:

   1. (a) Circumstances leading up to the violation(s);
   2. (b) A description of the violation;
   3. (c) Response by staff;
   4. (d) Response by the juvenile(s) involved in the incident; and
   5. (e) Sanctions imposed or recommended for the violation(s).

2. (2) Service providers must:

   1. (a) Forward all incident reports to the JRA program administrator no later than twenty-four hours after the behavior is discovered; and
   2. (b) Verbally report serious violations to the JRA program administrator immediately.
